• Be a business located in the United States 
• Have an active professional medical license: ND, DMD, OD, MD, DO, DC, DOM, PharmD, APRN license 
• Hold a tax exemption certificate. Without that, we are legally obligated to charge tax on applicable items. The name on the certificate should match the company name associated with your account.
